If you are experiencing any mental or emotional symptoms, such as depression or anxiety you may want to fill in the online Anxiety and Depression Questionnaire before your next appointment. The results from the questionnaire will help me to gain a better picture of your health pattern, thus enabling a better treatment outcome.

The questionnaire will take up to an hour of your time to complete, so please ensure that you give yourself enough time before starting. You will need to register first by clicking "New patients click here to get User ID". A password will be sent to you by email before you can start the questionnaire.

Please make sure that you complete the questionnaire at least two days before your appointment
